Here, Drs. Loveless and Stevens describe this process and how doctors can make sure they follow it. When starting out you should ask: is there evidence to support this? Looking towards stakeholders and providers is also important if an LCD is needed. Systematic approaches are also critical, especially when it comes to study design.
