Dr. Antonio Rezoagli begins with a review of the anatomical locus for cervicogenic headache, including the intersection of sensory nerve fibers from the trigeminal nucleus and the upper cervical roots, and their contributions to referred pain. He moves on to the epidemiology and differential diagnosis of cervicogenic headache, including approaches to diagnosis and the use of anesthetic blockade to confirm a diagnosis. Dr. Rezoagli concludes by considering standard and experimental treatment options for cervicogenic headache.

Dr. Amol Soin describes the results of a study on the use of a patient navigator. Dr. Soin thinks we can use digital technologies to improve patients’ experiences. Dr. Soin and his team investigated the use of a Patient Navigator to improve opioid compliance. They found that the best predictor of noncompliance was depression and anxiety. Navigator group patients also had lower pain scores, lower morphine milligram equivalents (MME), and lower discharge rates. However, causality still needs to be determined.

Dr. Alex D. Pham discusses pharmacological advances in opioid therapy and the role of oliceridine. As the number of patients with pain has grown, so has the need for better pharmacological treatments. Oliceridine provides a compelling alternative because this compound that binds to mu-opioid receptors but are coupled to a Beta-arrestin inhibitor. This allows for analgesia with a reduced side effect profile. Overall, a literature analysis supports the use of oliceridine for pain management.

Dr. Sairam Atluri discusses the current landscape in biologics in musculoskeletal and spinal disorders. Now, according to the FDA, no stem cells or other biologics can be used at all outside of approved clinical trials. Dr. Atluri argues that stem-cell based interventions are not as negative as some reviews suggest, and instead complications may be due to medical practitioners operating outside of their scope of training.

Dr. Alex D. Pham describes how spinal cord stimulation can be used for painful diabetic peripheral neuropathy. However, traditional treatments are oftentimes discontinued due to poor tolerability, low patient satisfaction, etc. What role is there for spinal cord stimulators for diabetic peripheral neuropathy. A large database review demonstrates that clinical studies indeed support the use of spinal cord stimulation to improve pain and patient outcomes.

Dr. Alex D. Pham discusses the dorsal root ganglion (DRG) and chronic pain. Chronic neuropathic pain is widespread and difficult to treat. However, targeting the DRG has begun to show promising effects in pain alleviation. Dr. Pham and his colleagues conducted a systemic literature search and found several promising case reports that support the use of DRG stimulation. Overall, this developing technique may allow for a precise approach to chronic pain management.
