Andrea Trescot, MD, demonstrates cryoanalgesia (cryo) for several pain conditions on a cadaver at a World Institute of Pain (WIP) workshop in Miami, FL. Dr. Trescot mainly discusses performing cryo on the infrapatellar saphenous nerve for knee pain. She also discusses other applications including relieving pain from knee replacements, deep vein thromboses (DVTs), diabetic neuropathy, and phantom limb pain. She discusses the different nerves that are targeted for the different applications.

Dr. Hemmo Bosscher discusses the use of PACIF (Percutaneous Ablation, Curettage and Inferior Foraminotomy) to treat common low back pain at the 24th Annual Gabor Racz Pain Conference and Workshop. The treatment of nonspecific lower back pain can be difficult because it is, by definition, not localized to a particular area. However, the newly developed PACIF system may allow for better pain treatment outcomes.

Prof. Marshall Devor discusses the science underlying interventional lumbar dorsal root ganglion (DRG) procedures at the 24th Annual Gabor Racz Pain Conference and Workshop. He first describes the importance of identifying where pain comes from and the critical role of the DRG in ectopic pain generation. Targeted suppression of ectopic pain generation in the DRG may prove to be one therapeutic option in pain management for phenomena such as phantom limb syndrome. Prof. Devor’s talk also illustrates the importance of animal models in pain research.

Dr. Kenneth Chapman describes the stellate ganglion and T2/3 sympathetic block at the 24th Annual Gabor Racz Pain Conference and Workshop. The sympathetic nervous system is critically tied to the stellate ganglion. Because of this, there are many indications for stellate ganglion blockade, from frost bite to post-traumatic stress disorder. However, there are potential complications to be aware of, such as infection. There are also several different approaches to stellate ganglion block, included ultrasound guided. T2-T3 sympathetic block is also touched on here.

Dr. George Chang Chien discusses the role of cannabidiol (CBD) in pain management at the 24th Annual Gabor Racz Pain Conference and Workshop. CBD is a non-psychoactive compound found in the cannabis plant and has been used in pain management for centuries. It is both an anti-inflammatory and analgesic. CBD exerts its effects through the endogenous cannabinoid system within the brain and body. Dr. Chien discusses the mechanisms by which CBD may have its pain-relieving properties here.

Dr. Douglas Beall discusses current state of technology in diagnosing discogenic pain at the 24th Annual Gabor Racz Pain Conference and Workshop. There are different ways of treating discogenic pain, such as fusion surgery, but spine surgery for back pain doesn’t have very high success rates. In fact, Dr. Beal doesn’t even perform fusion surgeries for discogenic back pain. Instead, he uses a combination of stem cell therapy and nerve ablation procedures. Other methods are available, and Dr. Beal discusses them here.

Dr. Mert Akbas discusses lumbar joint facets at the 24th Annual Gabor Racz Pain Conference and Workshop. After diagnostic procedures, lumbar facet pain can be treated with one of several techniques, most notably medial branch blocks and percutaneous radiofrequency medial branch neurotomy. However, these surgeries are delicate, and care must be taken, such as Dr. Akbas describes here.

Dr. Vikram Patel discusses cervical spine pain at the 24th Annual Gabor Racz Pain Conference and Workshop. Anatomical considerations are critical when treating cervical spine pain. Many types of injuries can cause cervical spine pain, and clinical presentations can vary widely. There are also different types of interventions to help reduce pain, as Dr. Patel describes in more depth here.

Dr. Kenneth Chapman discusses percutaneous vertebral augmentation at the 24th Annual Gabor Racz Pain Conference and Workshop. The cost of vertebral fractures is high, particularly for women with osteoporosis. However, there are procedures that may be able to help treat the pain of vertebral compression fractures. Surgical vertebral augmentation may help stabilize these fractures and prevent micromovements. Decisions to treat with vertebral augmentation takes a multidisciplinary team, and patients and their families need to be properly informed about the very technical procedure.
