Dr. Chris Gilligan discusses restorative neurostimulation for mechanical chronic low back pain. He illustrates what impaired motor control and atrophy of multifidus and longissimus look like in the brain for people with pain. Impaired control of the muscles leads to a vicious cycle of functional instability and chronic pain. However, there is hope for rehabilitation. Stimulation of the medial branch of the Dorsal Ramus to induce multifidus contractions can be used to restore neuromuscular control. The data from Dr. Gilligan’s ReActiv8-B trial are promising and support the use of restorative neurostimulation approaches.

Dr. Juan Carlos Flores discusses radiation safety. There are many sources of radiation in our everyday environments, particularly in the clinical setting. Dr. Flores discusses x-rays, a prominent form of radiation for pain specialists, in depth. He argues that we should be more responsible managing the interaction between x-rays and living matter. Fluoroscopy use also needs to be justified. Radiation has been linked to cancer. Thus, reducing radiation exposure for everyone is critical.

Dr. Andre Mansano presents on epidural transforaminal techniques and the differences between them. The Safe Triangle approach (subpedicular technique) and Kambin’s Triangle approach (intraneural approach) are two of the methods that are discussed. Safety is also an important topic when discussing epidural approaches. Research in the field of epidural transforaminal techniques are discussed, and examples are also used.

Dr. Vikram Patel discusses ways to optimize radiation protection. Protecting patients, doctors, and staff from radiation is incredibly important. There are many hazards of radiation to the human body. However, there is a need for radiation in pain work, so how can radiation be minimized? Positioning and protection are key. Reducing time of exposure or reducing output can help. It is also to check exposure levels over time, as radiation accumulates. These ideas and more are described.
