Targeted Drug Delivery for Chronic Pain: An Underutilized Therapy

Dr. Schultz presents the evolution of interventional pain management, followed by the standard interventional algorithm for pain management. He further explains that if several procedures fail, including surgery, the next recommendation is neuromodulation through neurostimulation or targeted drug delivery (e.g., pain pump). He mentions that neurostimulation is typically performed before targeted drug delivery, which is generally the last resort. Dr. Schultz also explains that if certain limitations of neurostimulation (e.g., electrode placement, region specific, failure rate) arise, target drug delivery should be considered. Targeted drug delivery provides diffuse drug distribution when other approaches have failed.
