Extruded disc & epidural injection

      Hi dear colleagues
      Are we allowed to do epidural block in patients with extruded disc who aren’t ready or there is no enough indications to go under open surgery ? is there any differences between TFESI and interlaminar epidural ?

        Hello and good evening. I am not certain by what you mean by “are we allowed”. If a patient has an extruded disc and is having pain, I will see then and perform a thorough exam. I will look at their MRI. Many of such patients are referred to me by neurosurgery or ortho spine. If I determine that their pain generator is the extruded disc, I will initially start with an inter laminar epidural steroid injection placed either one level above or one level below the extruded disc. If it is not effective, I will try a TFESI at the extruded disc site/level. Usually, the interlaminer epidural works well sometimes I have to repeat it once.
